Why Your Heel Hurts Most in the Morning | HWY PT

You know the moment. Your feet hit the floor, you put weight through the first heel, and something in there objects. So you put a hand on the wall. You take the first few steps on the outside edge of the foot, or on your toes, and you shuffle to the bathroom like someone twice your age.

Then, after a minute or two of moving around, it fades. By mid morning you have almost forgotten about it. That is the part that confuses people. If it hurt all day, you would have called someone weeks ago.

Most people assume this is plantar fasciitis, and most of the time they are right. But not always. Three other problems produce heel pain in adults over 50, and two of them get worse if you treat them like plantar fasciitis.

This article is a sorter, not a treatment plan. It walks you through the four real causes, gives you a self test you can do sitting in a chair, and tells you which one your pattern points toward.

First, the short list that needs a doctor sooner

Most heel pain is mechanical and not urgent. A few situations are different. Please book with a physician or an urgent care clinic promptly, rather than waiting it out, if any of these apply to you.

  • Your heel started hurting after a fall, a hard landing, or a sharp jump in how much you were walking, and it is getting worse instead of better.
  • The pain is constant, does not settle at all with rest, and it is waking you repeatedly.
  • The heel is red, hot, or swollen, or you have a fever.
  • Numbness is spreading from the heel into the rest of the foot or the toes.
  • You have diabetes and there is any open sore, blister, or break in the skin on that foot.

The American Academy of Family Physicians lists infection, including diabetic ulcers and bone infection, among the non mechanical causes of heel pain. That is why an open sore on the foot of someone with diabetes is a same week question for a physician, not a stretching question.

One honest caveat on night pain. Nerve related heel pain is often worse at night by itself, so waking up once is not automatically alarming. The concern is pain that never eases with rest, especially alongside redness, heat, or fever.

What your heel is actually doing at 6 in the morning

There is a thick band of tissue running from your heel bone forward along the sole of your foot. When it tightens, it pulls the bones of your midfoot together and locks your arch into a stiff lever you can push off from. StatPearls calls this the windlass mechanism, and describes it as the fascia pulling the midtarsal bones into a stable arch that supports your body weight.

Overnight, your foot rests in a relaxed position and that band sits short. Your first steps stretch it suddenly, under your full body weight, on a cold floor.

That is why the timing of your pain is such useful information. Which tissue objects, and at which moment, tells you a lot about which of the four problems you have.

Cause one: plantar fasciitis

This is the common one. AAFP states plainly that plantar fasciitis "is the most common cause, with a lifetime prevalence of 10% in the general population." StatPearls puts it at roughly 2 million Americans a year.

The signature is specific. AAFP describes "throbbing medial plantar heel pain that is worse with the first steps after rest." Medial means the inner side. Plantar means the sole. So the sore spot is toward the inner front corner of the heel, where the band anchors to the bone.

Three features tend to travel together.

  • It is worst on the first steps after your foot has been still for a while, and it eases as you keep moving. AAFP notes the pain "often decreases after further ambulation, but can return with continued weight bearing."
  • It comes back at the end of a long day, especially after standing or walking a lot.
  • It has one tender point, not a broad sore area. Pressing over that inner corner of the heel bone produces what AAFP calls "sharp, stabbing pain."

Who gets it: APTA's guideline summary in American Family Physician reports that it most commonly affects people aged 40 to 60. APTA's patient guide says it "occurs most frequently in people in their 40s, but can occur in all age groups." StatPearls lists obesity, prolonged standing, and unsuitable footwear as risk factors, and states that men and women are affected equally.

Cause two: the heel fat pad has thinned

Underneath your heel bone sits a shock absorbing cushion of fat, built in chambers like a honeycomb. It does not stay the same forever. Cleveland Clinic states it simply: "Your heel loses some of its fatty tissue and elasticity as you age."

The pain has a different character and a different address.

  • Location: the middle of the heel, not the inner corner. Cleveland Clinic says the pain "is closer to the center of your heel," while plantar fasciitis pain sits "closer to your instep or inner part of your heel."
  • Quality: AAFP describes it as "a deep, bruise-like pain, usually in the middle of the heel." People often say it feels like the padding is gone, or like they stepped on a stone.
  • Trigger: hard surfaces and bare feet. StatPearls describes diffuse central heel pain "often precipitated by barefoot walking on hard surfaces."

AAFP names three contributors: "Decreased heel pad elasticity from aging, prior corticosteroid injections, or increased body weight." That middle one matters. StatPearls notes that a corticosteroid injection "poses a mild risk of fat pad atrophy or plantar fascia rupture."

So if you have had injections into that heel already and the pain has changed character from sharp and localized to deep and central, that change is worth mentioning to whoever treats you next.

Cause three: a calcaneal stress fracture

This is the one that gets worse if you push through it. A stress fracture is a small crack in the heel bone from repeated loading rather than a single injury.

StatPearls describes the mechanism as "repetitive submaximal loading that exceeds the bone's ability to remodel." It distinguishes two kinds. Fatigue fractures happen in normal bone under too much load. Insufficiency fractures happen in weakened bone under ordinary load, which is the more relevant one after 50.

The history is the giveaway. AAFP: "Pain usually begins after an increase in weight-bearing activities or after changing to a harder walking surface."

Think about the six weeks before it started. A new walking program. A trip with a lot of pavement. A move to a house with tile floors. A new job on your feet.

Two more features separate it from plantar fasciitis.

  • The direction of travel is wrong. AAFP notes the pain "initially occurs only with activity, but it can later occur at rest." Plantar fasciitis tends to settle once you are moving. A stress fracture tends to build the longer you are up.
  • The squeeze test. AAFP: "A positive calcaneal squeeze test (i.e., pain on squeezing the sides of the calcaneus) suggests the diagnosis." StatPearls describes the same finding as tenderness on compressing the heel bone from both sides.

StatPearls lists osteopenia, corticosteroid use, and sudden increases in training among the risk factors, and warns that "continued weight-bearing may result in fracture propagation or displacement." It also notes these injuries "generally follow a more favorable course if recognized early."

One thing to know before you ask for an X ray. AAFP states that "radiography often does not reveal a fracture, so bone scans, computed tomography, or MRI may be required." A normal X ray does not settle the question.

Cause four: a pinched nerve in the ankle or arch

The fourth cause is not soreness at all. It is a nerve being compressed, and it produces a different vocabulary.

AAFP describes tarsal tunnel syndrome as compression of the posterior tibial nerve, causing "burning, tingling, shooting pain, or numbness in the posteromedial ankle and heel." StatPearls defines it as an entrapment neuropathy of the tibial nerve beneath a band of tissue on the inner ankle.

A smaller branch can be pinched further along. StatPearls names it: "The 1st branch of the LPN, known as the Baxter or inferior calcaneal nerve." Pain from that branch is described as "sharp, radiating pain," worse after walking and at night.

Listen to the words you naturally reach for. Sore, tender, and bruised point toward the first three causes. Burning, buzzing, tingling, electric, and numb point toward a nerve.

Timing helps too. StatPearls reports that tarsal tunnel symptoms "may worsen at night, during prolonged walking or standing, or after physical activity," and that they "typically improve with rest." That is close to the opposite of the first steps pattern.

To be clear about scope: this is a local nerve compression at the ankle or arch. It is not the same thing as widespread nerve damage affecting both feet, which is a separate topic with separate causes.

A five step self test you can do in a chair

Do this sitting down, in good light, with the sore foot resting across the opposite knee or on a low stool. Nothing here requires you to stand on it, hop, or force anything. Go gently. If a step hurts sharply, stop that step and note it as a finding.

Step 1. Find the inner corner. Run your thumb along the sole until you reach the front inner edge of the heel bone, roughly where the heel meets the arch. Press firmly for two seconds. Note whether you get a sharp, pinpoint pain.

Step 2. Press the middle of the pad. Move your thumb back to the center of the heel, the part that hits the floor first. Press with the same pressure. Note whether this feels deep and bruised rather than sharp.

Step 3. Tension the band. With your other hand, pull your big toe gently back toward your shin and hold it. You should feel a cord tighten along the sole of the foot. With the toe still held up, press along that cord. Note whether pressing it now hurts more than it did in step 1.

Step 4. Squeeze the sides. Place your thumb on one side of the heel bone and your fingers on the other, well behind the tender spot from step 1. Squeeze gently, like testing a ripe peach. Note any deep ache that comes from inside the bone.

Step 5. Tap behind the inner ankle bone. Find the bony bump on the inside of your ankle. Just behind and below it, tap lightly with a fingertip, five or six times. Note any tingling, buzzing, or electric feeling that shoots into the arch or the toes.

Now read your findings against this table.

Three honest warnings about this table.

First, these problems coexist. A thinned fat pad and an irritated fascia commonly sit in the same heel, and a nerve can be irritated alongside either.

Second, a gentle squeeze that hurts is a reason to see a clinician, not a reason to start treating a fracture yourself. It is a clue, not a diagnosis.

Third, no home test replaces an examination. AAFP notes that imaging "is not required" to diagnose plantar fasciitis, which is exactly why the history and the hands on exam carry so much weight.

What the guideline actually recommends, and how strong the evidence is

The physical therapy profession publishes a graded guideline for this. The current version is the 2023 revision by Koc and colleagues in the Journal of Orthopaedic and Sports Physical Therapy. APTA states that it covers non arthritic heel pain in people aged 16 and older.

Here is what that guideline reports, as summarized by American Family Physician in 2025.

  • Stretching the plantar fascia and the calf muscles reduces pain and disability. AFP is careful about the strength of that finding: "Low- to moderate-quality evidence supports the benefit of stretching the plantar fascia and the gastrocnemius and soleus muscles."
  • Strengthening the toe flexors, the muscles that turn the foot in and out, and the calf "improve pain and function more than stretching alone."
  • Hands on treatment helps, but for defined windows. AFP lists subtalar joint mobilization improving pain for up to 3 weeks, deep massage and nerve mobilization up to 6 weeks, and instrument assisted soft tissue work up to 90 days.
  • Dry needling of the calf and foot muscles "improves pain and reduces disability for up to 6 months."

On the examination side, the same summary describes the classic picture as plantar medial heel pain that worsens with the first steps after a period of inactivity, together with tenderness where the fascia attaches, a positive windlass test, and limited ankle motion.

About the letter grades

The guideline rates each recommendation with a letter. Grade A means strong evidence, B moderate, C weak, D conflicting, E theoretical, and F expert opinion.

We could not obtain the 2023 graded table, because it sits behind a journal subscription. So we will not print 2023 grades we have not read.

What we can tell you is how the previous revision graded things, as published in the APTA guideline summary. Manual therapy, stretching, antipronation taping, foot orthoses, and night splints all carried grade A. Low level laser was C. Electrotherapy was D, meaning the studies disagreed. Patient education was E, strengthening was F, and trigger point dry needling could not be recommended.

Compare that with what the 2025 summary of the newer revision reports about strengthening and dry needling, and you can see the evidence has moved. We are describing two documents, not claiming a specific grade changed.

The honest limits

An independent group published a best practice guide in the British Journal of Sports Medicine in 2021, built from a systematic review, expert interviews, and patient values.

Their core recommendation is close to APTA's: treatment should include taping, stretching, and individual education, with shockwave therapy and custom orthoses as options for people who do not improve enough.

But look at the arithmetic they report. Fifty one eligible trials enrolled 4,351 participants, and only nine randomized trials were good enough to determine proof of efficacy for ten interventions. That is a thin evidence base for a very common problem, and it deserves saying out loud.

Two more things worth knowing before you set your expectations.

The natural history is good but it is not quick. StatPearls reports that "up to 90% of patients will recover with conservative care," and in the same breath that "the average duration of chronic heel pain can persist for just over one year."

And APTA's patient guide states that most cases improve over time with conservative treatment and that surgery is rarely needed. That is genuinely reassuring. It also means patience is part of the treatment, which is not what most people want to hear in week three.

What an evaluation for heel pain actually looks like

If you have never been to physical therapy, here is the shape of a first visit for this problem. In Oregon you can see a physical therapist directly, without a physician referral first.

  1. The history. When in the day does it hurt, how long have you had it, what changed in the weeks before it started, what surfaces and shoes you spend your time on, and what you have already tried.
  2. Looking and measuring. APTA's patient guide describes observing, measuring, and assessing the position of your foot, ankle, and arch, standing and sitting.
  3. Hands on examination. The same guide describes palpation of the heel and arch. This is the step that separates a pinpoint fascia insertion from a diffuse central pad, and it is difficult to do properly on yourself.
  4. Movement testing. Ankle motion, calf flexibility, and how your foot loads when you walk. Restricted ankle dorsiflexion is part of the classic picture in the guideline summary.
  5. Screening the other candidates. Nerve tests, a squeeze of the heel bone, and questions about your general health, so that a fracture or a nerve problem is not missed.
  6. A plan with a review date. What you will do, how often, and when we look again to see whether it is working. If you want to know how to judge that, read how to tell whether your physical therapy is working.

A note on our own equipment, because people ask. If you are reading this for a parent

You may have noticed it before they mentioned it. The hand on the dresser in the morning. The slow first lap to the kitchen. The shoes that never come off indoors anymore.

Three things are worth doing, in this order.

Ask when, not whether. Do not ask if their foot hurts, because the honest answer is often no by the time you are talking. Ask what the first steps out of bed are like, and what the last hour of the day is like.

Ask which word fits. Sore and bruised point one way. Burning, buzzing, and numb point another. That single word changes the direction of the workup.

Ask what changed. A new walking habit, a different floor, a trip, worn out slippers. A heel that has hurt for three weeks after a change in load is a different conversation from a heel that has grumbled for three years.

If they have diabetes, check the skin of the whole foot while you are there, including between the toes and the back of the heel. Any open area goes to a physician promptly.

Questions we get asked

Is morning heel pain always plantar fasciitis? No, but it is the most likely single cause. AAFP calls it the most common cause of heel pain, with a lifetime prevalence of 10%. The pattern that fits best is pain on the inner front corner of the heel that eases as you move and returns after a long day. Central, bruise like pain and burning or tingling pain point elsewhere.

Should I keep walking, or rest it? That depends which cause you have, which is the whole point of sorting first. For a suspected stress fracture, StatPearls is explicit that continued weight bearing can make the fracture worse, so that one needs a physician before you add any more miles. For the others, complete rest is rarely the answer, and a therapist can help you find the level of activity that does not provoke it.

I had a normal X ray. Does that rule out a fracture? Not on its own. AAFP states that radiography "often does not reveal a fracture, so bone scans, computed tomography, or MRI may be required." If your history fits a stress fracture and the pain is still building, say so to your physician even after a clear X ray.

How long does this take to settle? Longer than most people expect. StatPearls reports that up to 90% recover with conservative care, and that chronic heel pain averages just over a year in duration. In practice we set a goal and a review date at the first visit, usually a few weeks out, so you are judging progress against something specific rather than against hope.

Do I need a referral from my doctor to see a physical therapist in Oregon? No. You can book a physical therapy evaluation directly. What if it is a mix of two things? That is common, and it is a normal finding rather than a complication. A heel can have an irritated fascia and a thinned pad at the same time. The examination sorts out which one is driving the pain today, and the plan addresses that one first.
